Request documentation is required and should include the patients:
Name
Address
Telephone number
Date of birth
Tests requested
Doctor’s name, address and telephone number
Clinical Notes
Name and address of person or department responsible for the account.

10mls of blood in EDTA and/or 10mls of blood in Lithium Heparin are required for testing.

Send by overnight courier so as to arrive within 24hrs of collection to:

Please do not collect or send blood samples on Thursdays or Fridays in case of delays.

If sending DNA the DNA is to be extracted from 10mls of EDTA or Lithium Heparin blood - please include method of extraction and if the DNA is in TE. Please send the DNA to the above address.

All tests will take approximately six(6) to eight(8) weeks from time of receipt of the blood or DNA sample.

Sorry there is no Medicare rebate available, GST must be added to all test charges.

Print a copy of these conditions and send with any shipping documentation accompanying the sample.

DNA does not require refrigeration or freezing in dry ice so may be sent by normal mail BUT the sample must be packed in accordance with international IATA requirements concerning safe handling, transport and labeling of biological material.
